A 15:00 kick-off, for a player who trains hard and does not want to feel heavy. Times are from kick-off; the amounts are worked out per kilogram in the plan, not guessed here.

Anthropometric
Height, mass, and skinfolds taken by somebody accredited to take them. Repeated the same way every time, by me, so the numbers are comparable rather than merely collected.
Biochemical
Bloods, where your doctor has them or where they are worth requesting — iron status and vitamin D most often. I read them; I do not order them, and I refer back when something is outside my scope.
Clinical
Injury and illness history, medication, gut symptoms, menstrual status where relevant. The part that most often explains a problem everybody else has been treating as a diet problem.
Dietary
What you actually eat, over days that look like your real week rather than your best one. Weighed where it matters, estimated where it does not.

Assess before advising
The ABCD framework, in full, before a plan exists. Anyone writing you a meal plan in the first session is writing it for somebody else.
Fit the plan to the week you actually have
A plan that assumes you cook twice a day will not survive a Tuesday. Most of my job is finding the version you will still be doing in March.
Say what the evidence does not support
Including when that means telling you the thing you came in asking for does not work. It is the whole reason to see somebody registered.
