Back to playing, not back to comfortable
Hamstrings, ACLs and ankles. Rehabbed back to playing rather than back to comfortable.

Hamstring strain — The phases
A moderate strain, on a plan. The dates move; the order does not.

Return-to-play criteria
See the detail →Decided on measured criteria, not on whether it still hurts. Published here so you can hold me to it — and so your club can.
Hamstring strain
3 to 8 weeks| Test | Threshold | Measured by |
|---|---|---|
| Eccentric strength | ≥ 90% of the other leg | Hand-held dynamometer |
| Single-leg bridge | ≥ 90% symmetry | Repetitions to failure |
| Sprint exposure | 2 sessions at 95%+ max speed | GPS or timing gates |
| Askling H-test | No apprehension | Clinical |
Limb symmetry at week 6. Under 90% is not a pass, and the eccentric number is the one that matters most here.

Assessment
€6545 min
What happened, what it is, and how long it will realistically take. You leave with a plan and a timeline, in writing.
Follow-up
€4530 min
Treatment and progression. Most people need between four and eight of these, and I will tell you when you no longer do.
Return-to-play testing
€12060 min
The full battery against the published criteria. Useful before you go back, and before a club signs you off.

Patients
Three people, and what actually happened. Including the part where they disagreed with me.
I had torn the same hamstring three times in two seasons. The difference here was that he told me week six and meant it, and would not sign me off at week four when I asked. First season since 2021 I have finished.Hamstring · 3rd recurrence · returned week 7
My surgeon said six months. Rodrigo showed me the criteria, wrote to him, and the two of them agreed on nine. I was furious at the time. My quad was at 74% in month six, which I would never have known.ACL reconstruction · returned month 10
I had rolled the same ankle so many times I thought it was just my ankle. Six weeks of balance work that felt pointless at the time and it has not gone since. Nobody had ever tested it.Chronic ankle instability · no recurrence in 14 months

How I work
See the detail →Load it, do not rest it
Beyond the first few days, rest is what makes tissue weaker. Almost all of the work is loading — carefully, progressively, and earlier than most people expect.
Measure, do not ask
“How does it feel” is not a return-to-play criterion. Strength symmetry, hop tests and movement quality are, and they have numbers attached.
Say the timeline on day one
Even when it is sixteen weeks and you do not want to hear it. The people who reinjure are the ones who were told what they wanted.
